CHRONIC DISEASE MANAGEMENT

PROGRAMME

In  Ireland today over one million people suffer from a Chronic Disease. These are diseases which are persistent and long lasting in their effects. They include certain heart and lung diseases and diabetes. 

Many people attend hospital out-patient services and their GPs for the management these diseases. A more co-ordinated, structured approach to dealing with these diseases will help improve management and prevent complications of these diseases. 

The HSE has established an Integrated Care Programme for the Prevention and Management of Chronic Disease. This programme aims to provide better care to people with certain long term illnesses. This will be achieved by providing access to preventative, management and support services to patients with these conditions.
