PROGRAMME (CDM)

In  Ireland today over one million people suffer from a long term condition. These are conditions which are persistent and are long lasting in their effects. They include certain illnesses  such as COPD, Asthma, heart disease and diabetes. 

The HSE has established an Integrated Care Programme for the Prevention and Management of Chronic Disease. This programme aims to provide better care to people with certain long term illnesses. It allows for care to take place predominantly in a general practice setting and not in hospital outpatients. This will be achieved by providing access to preventative, management and support services to patients via their GP.

Deansgrange Medical Centre is taking part in this programme. If you qualify to participate, our nurses will contact you to invite you to join. There is no charge for the service. For you it will involve visiting the surgery for blood tests once a year and nurse and doctor consultations approximately twice a year. We can also issue your repeat prescriptions during your visits.
